Long Term Treatment of Cleft Lip and Palate Patient with Excessive Mandibular Growth

Abstract
One of the typical facial growth pattern after reconstruction surgery is retruded maxilla due to the surgical scar. Therefore, orthopedic treatment of cleft lip and palate patient is necessary for most of the cleft cases. Facemask is the choice of the orthopedic appliances for retruded maxilla. In this case a male patient with right side unilateral cleft lip and palate has been gone through phase I treatment with maxillary expansion and protraction and phase two with extraction and class III mechanics. Since the patient has mental retardation, behavior management and control was very demanding. However, no professional care for psychiatric aspect was involved in this case. Although maxillary expansion was successful, wearing order for the facemask was not done well. The phase two treatment could not achieve the goal of the treatment which is the formation of the positive overjet. The appliance was removed by request from the patient and his parents.
